In-person visit
Provider was great, booking was great. But they tried to bill my insurance in the morning, hours before my appointment or any service was completed. How do they know if I need to cancel last minute? Or add/remove services? Then trusting that they remember to "void" the transaction? Not OK, and likely won't be back because of that.
