Description
We require a clean unused medical mask to be worn in the clinic. We can provide one to you for 50 cents. Please enter the clinic wearing your own mask and then CHANGE YOUR MASK ONCE INSIDE. If you bring your own disposable, medical mask, it must be in its packaging or a ziploc bag ONLY. The mask should be placed over your mouth and nose at all times. Cloth masks will not be permitted A COVID Screening Survey will be emailed to all individuals who are having an in-person session, 24 hours before your appointment. This must be completed immediately upon receipt and BEFORE arriving to the clinic
